1. BI-02D.02 Standards that set minimum requirements for cabin integrity, restraints, access and escape, auto-shut downs and other relevant protective systems - Expectation
Equipment selected for site use is fit for purpose and delivers safe and productive outcomes.
How do we currently meet this expectation? Is it relevant to us?
If No to question 1 above please provide details
Why doesn't BI-02D.02 expectation not apply?
1.1 - Specification
Please provide details of where the specification can be found. Provide specific references, that is text which requires this Business Input - not just a document name. Mobile equipment specifications are in place that include requirements for:
- Cabin integrity and operator restraint (seat belts)
- Auto shutdowns
- Ergonomics of control panels/devices
- Warning devices e.g. tilt switches, proximity alarms
- Protective devices for emergencies (fire, collision, etc.)
- Egress / escape by alternate exit in the event of an emergency e.g. through non-laminated glass windows
- Required ratings for Light Vehicles
- Confirming that statutory conformance requirements are met for Heavy Vehicles e.g. ROPS / FOPS and head/whiplash protection.
- As required the installation self-rescuers for vehicles that may be engulfed e.g. dozers that operate on stockpiles
1.2 - Implement
Describe how the Business Input is implemented. Is it adequate, consistent, reliable, and repeatable? Do people know what is expected of them?
Senior managers including representative of operations and maintenance approve equipment selection recommendations.
A rigorous introduction to site process checks that received mobile equipment meets site requirements before it is permitted to operate.
1.3 - Monitor
Please provide details of how the status of the Business Input is monitored.
Comments could address how standards are kept current with emerging requirements, maintenance strategies to address requirements, and oversight of role holders who document the site requirements for mobile equipment.
1.4 - List relevant documents
Please list all relevant documents.
1.5 - Attach relevant documents
Please attach relevant documents.
2. BI-02D.04 Processes for confirming that new to site vehicles meet site specifications including vehicle lights, signage and markings (Intro to Site) - Expectation
All site vehicles in operating areas meet specifications for vehicle reversing & head lights, running lights, indicator and identification numbers.
Is the expectation of this Business Input relevant in your circumstances?
If No to question 2 above please provide details
Why doesn't BI-02D.04 expectation not apply?
2.1 - Specification
Please provide details of where the specification can be found. Provide specific references, that is text which requires this Business Input - not just a document name. Ordering / selection of equipment considers site requirements for lights and markings. The Introduction to Site process for mobile equipment checks lights, vehicle markings, mirrors/cameras and any technology in place to aid vehicle operators that was specified for the machine type.
Additional confirming checks are made on all aspects of the vehicle to confirm that the machine meets site specifications, comes with supporting documentation of conformance to requirements, and is suited to maintenance and operating (including emergency response) capacities on site.
2.2 - Implement
Describe how the Business Input is implemented. Is it adequate, consistent, reliable, and repeatable? Do people know what is expected of them?
Introduction to site processes include documenting status of the vehicle and confirming it meets site requirements for load carriage/towing, braking/steering, critical systems, lights, signage and markings.
Maintenance strategies for new to site plant include raising work orders for all related systems, components, lights, signage and markings.
2.3 - Monitor
Please provide details of how the status of the Business Input is monitored.
Comments could address development of maintenance strategies for all plant on site, inclusion of contractor equipment in site registers, and audits of deployed equipment vs maintenance strategies in the CMMS.
2.5 - Attach relevant documents
Please attach relevant documents.
2.4 - List relevant documents
Please list all relevant documents.
3. BI-02P.11 Protocols that detail how the status of vehicle alarms and monitors are checked prior to use - Expectation
Vehicle operators check and report on the status of vehicle alarms before use.
Is the expectation of this Business Input relevant in your circumstances?
If No to question 12 above please provide details
Why doesn't BI-02P.11 expectation not apply?
12.1 - Specification
Please provide details of where the specification can be found. Provide specific references, that is text which requires this Business Input - not just a document name. There is a site requirement for vehicle operators confirm that alarms and monitors are working on start up. This may be achieved through equipment self-checks.
12.2 - Implement
Describe how the Business Input is implemented. Is it adequate, consistent, reliable, and repeatable? Do people know what is expected of them?
Vehicle operators are competent to check alarm and monitor status prior to use.
12.3 - Monitor
Please provide details of how the status of the Business Input is monitored.
Comments could include specifications for equipment that include self-test on start-up as part of site specifications/requirements, competency checks on operator's pre-start understanding, maintenance record oversight for safety related work orders.
